Printed certified copies of birth certificates are available at the Health Department for those who were born in Missouri. Birth Certificates are $15 each and are available Mon-Fri from 8:00 a.m.- 4:00 p.m..

Photo I.D. is required for the person requesting the certificatre. Persons whom may request a certificate are: Self, Mother, Father, Wife, Husband, Daughter, Son, Brother, Sister, Grandmother, Grandfather, Grandchildren, Aunt, Uncle or Nephew.

Printed certified copies of death certificates are available at the Health Department for deaths which occured in Missouri after 1980. For those before 1980, death certificates are available through the State of Missouri Office of Vital Records.

Photo I.D. is required for the person requesting the certificate. Persons whom may request a certificate are: Self, Mother, Father, Wife, Husband, Daughter, Son, Brother, Sister, Grandmother, Grandfather, Grandchildren, Aunt, Uncle, Nephew or an attorney acting on behalf of the registrant of thie family.

Death Certificates are $13 for the first copy and $10 per additional copy ordered at the same time.
